What's the Purpose
| The purpose of an INTAKE is to determine if a person is Eligible to receive mental retardation services in Pennsylvania. |

| The Case Management Unit handles all INTAKES for the Mental Retardation Program of Dauphin County. |
What happens during an Intake?
| An INTAKE is the process of gathering necessary information ("taking it in"). In this case, the purpose of gathering the information is to determine if a person is ELIGIBLE to receive mental retardation services in Pennsylvania. |
| You will speak to someone (a Service Access Coordinator - SAC) from the CMU and you will be asked some basic questions about the person who wants to enroll in MR services. |

| A meeting will be scheduled. You will be asked to bring specific information to this meeting. You will also be given a number of different resource materials. |
| After the meeting things will continue to happen! |

| Remember that these contacts are the way that the CMU collects information to help determine ELIGIBILITY. |
What information should I bring?
| Please bring each of the following materials to the Intake Meeting. |
| All insurance cards | ||
| Educational records | ||
| Psychological reports | ||
| Current physical report from doctor | ||
| Birth Certificate | ||
| Social Security Card | ||
| Drivers License (if applicable) | ||
| Immigration papers (if applicable) | ||
| Financial Information - SSI, income, etc. | ||
| Other information that you have and feel will help the process of determining "eligibility" | ||
| If applicable, guardianship papers, power of attorney papers, and a copy of a living will |
| If you are determined to be eligible, Another Meeting will be arranged. |
What happens if I am determined: "Not Eligible"?
| It is possible that you may not be eligible for our services and here are some further steps that you may want to consider. |
| If the information we receive indicates you are not eligible, you will be notified and have a right to request a review if you do not agree with the determination of "not eligible." |

| Except for waiver services, appeals from a denial of eligibility follow the county administrative process designed for appeals under the Local Agency Law (2 Pa.
